Inspections
This should be completed after inspecting a unit at the
site being used for a public function. The original must be MAILED to
the State. A copy should also be included with the packet left at the
site with the unit.
This form should be also be complete monthly, listing all the units in
your inventory, noting they have been inspected by a Pa certified
inspector. Recommend that a copy be included with the packet left at the
site with the unit.
